About Dr. Pile-Spellman
John Pile-Spellman, M.D., F.A.C.R. is board certified in radiology. He is recognized as an international leader in interventional neuroradiology, specializing in the diagnosis, management, and treatment of cerebral aneurysms, strokes, tumors, vascular malformations, and the development of image-based services. Dr. Pile-Spellman has over 25 years of experience in developing high-impact, clinically relevant imaging and treatment paradigms through clinical care, research mentoring, and organizational leadership. He has been at the forefront of high-quality academic medicine, continuously adding to the knowledge and understanding of neurovascular diseases and striving to innovate and improve the management thereof. Dr. Pile-Spellman’s multidisciplinary approach allows him to provide the special care that he is known for, with great insight into the safety, outcome, and quality of life of the patient.
Prior to joining NSPC Brain & Spine Surgery (NSPC), Dr. Pile-Spellman was Vice-Chairman of Research and Director of Interventional MRI at Columbia University Medical Center, New York, NY. He continues his research activities at Columbia University’s School of Engineering in the Biomedical Engineering Department. He has been an attending radiologist and Director of Academic Interventional Neuroradiology at New York-Presbyterian Hospital.
Dr. Pile-Spellman is a member of more than 20 professional organizations and societies, including the American Society of Neuroradiology (ASN), the New York Academy of Medicine (NYAM), the New York Academy of Sciences (NYAS), and the American College of Radiology (ACR).
Education & Training
A graduate of the University of South Dakota, Dr. Pile-Spellman received his medical degree from Tufts University School of Medicine, Boston, MA. He completed both a residency in diagnostic radiology and a fellowship in neuroradiology at Massachusetts General Hospital, Boston, MA.
While on sabbatical from Massachusetts General Hospital, Dr. Pile-Spellman completed a second fellowship in interventional neuroradiology at New York University Medical Center, New York, NY. He was also a visiting fellow in endovascular neurosurgery at the Kiev Neurosurgical Institute, Kiev, Ukraine.
Publications
Dr. Pile-Spellman has published in more than 160 peer review journals and has served as a reviewer for many academic publications, including Stroke, The Journal of Cerebral Blood Flow and Metabolism, Radiology, The New England Journal of Medicine, and The Lancet. He has been an investigator in at least six pivotal, clinical trials for endovascular treatment, and has numerous patents for endovascular treatment devices and advanced diagnostic imagery.
Honors & Awards

Dr. Pile-Spellman’s many honors include membership in the Alpha Omega Alpha Medical Honor Society, selection as the recipient of the Saddek Hilal Faculty Research Award from Columbia University, and repeated selection by Castle Connolly Medical, Ltd. as one of “America’s Top Doctors” from 2001-2021. Dr. Pile-Spellman has also been included in New York Magazine’s “Best Doctors in New York” list from 2002-2016.
